Will assist in increasing data for the Family Planning Annual Report (FPAR). This is also an education opportunity of birth control methods available and allows for open discussion with clients. Teen Reproductive Health Survey 8.5" x 11" Poster: The goal of the survey (developed by the Kentucky Department for Public Health Family Planning Advisory Committee) is to assess current sexual and reproductive health knowledge, attitudes, and behaviors among Kentucky adolescents to better understand challenges in achieving reproductive well-being. The results of this survey will assist in the development of strategic actions to offer tools, resources, and information that will increase reproductive well-being across the state. Distribute to agencies in your community. 11. Contraceptive Educator Kit - (For LOAN - Order only 1 ): Resource that will enhance the educational experience by providing contraceptive materials for visual and interactive learning. Take to community events, education classes or training with clinic staff. 12. 2”x3.5” Family Planning Information Cards. Stack of 25 cards (padded) designed to be placed out in the community (gym, gas stations, bathrooms, shelters) for people to scan the QR code to locate Title X clinics or to tear off a sheet and leave the pad. The audience is intended for anyone in the community, with a focus for anyone who may need birth control, especially anyone who may not know where to obtain services, or be uninsured, underinsured, or may not realize they do have access to this healthcare service.
